I understand that the credit card left on file is automatically charged on the 1st of the month for $250.00.

I understand that I get a choice of ONE treatment per month included with the membership. The treatment choices are Laser Genesis, Diamond Glow, or a Chemical Peel.

I understand that the treatment must be used during the month and there are no rollovers and no exceptions.

I understand that the treatments are not transferable and must be used by myself.

There is a 4 month minimum commitment. This is ongoing until I request to terminate.

Termination must be given to Central Park Laser Aesthetics in writing at least 5 days before the end of the month via email at hello@cplanyc.com
